Why guess when it can be measured?

When prepping a tooth to recieve a Zirconia / PFM crown the biological perspective is to remove as little tooth substance as possible. This usually conflicts with required material thickness to achieve acceptable aesthetics and flexural strength.
Dr. Baker’s Patented, CNC precision milled, prep gauges are designed to measure TWELVE significant dimensions in everyday dentistry – accurately, every single time you need it.
It is important to accurately obtain 0.7 mm to 0.8 mm of finish-line width in Zirconia crown preps – too narrow margins or under-prepped would force the dental technician to produce a bulky, over contoured crown. Over preparation of the tooth potentially risks weakening the tooth structure and possibly inducing pulpal injury in a vital tooth.

When prepping for Zirconia Crowns it is quite important to obtain an inter-occlusal clearance of 1.5 mm with the antagonist tooth [ at MIP ]. Insufficient inter occlusal clearances results in weak prosthesis or a resultant hyper-occlusion in the final prosthesis.

These optimal numbers were obtained after years of lab-based and clinical research.

Clinicians usually rely on the bur-tip or a periodontal probe for verifying these measurements . Unfortunately a bur-tip / periodontal probe cannot be a reliable replacement. Here is why? The bur is a cutting instrument that undergoes wear and tear, thereby constantly reducing its cross-sectional diameter! Even when new and unused, the diameters of burs coming out of the factory are never consistent, varying significantly between manufacturers. A measuring device should be accurate and, reproduce consistently precise measurements, reliably.

When referring to measurements that are extremely small 0.5mm – 0.8mm, approximations could cause significant error values. Using CNC milled gauges, just like one would use feeler gauges, can certainly produce predictable & consistent outcome in preps.

Monolithic Zirconia crown preparations should have axial surface reductions of at least 0.5 to 0.8mm, and occlusal reduction depths of 1 to 1.5mm (Goodacre, 2004).
Ideally the depth of the cavity should just be 0.2 mm below the DEJ

Can I use Dr.Baker’s Prep Gauges for verifying the depth of depth-cuts ?

YES, the T1 tip has a laser marked depth ring at 0.5 mm specifically designed for verifying smaller depth cuts particularly for labial reductions. For verifying deeper depth orientation cuts, T1 depth ring at 1.0 mm could be used. T3 / T4 can be used as depth cut checks in occlusal reductions.

How can use Dr.Baker’s Prep Gauges for measuring inter-occlusal clearances ?

The best tip for verifying the adequate clearance values [1.5 mm ] Zirconia crown prep / inter occlusal clearance check is by using the T3 tip . Ask the patient to bite at MIP and gently push the the T3 / T4 tip head between the prepared tooth and the antagonist as show in the image alongside. T3 has a 1.5 mm diameter. For PFM preps where 2.0 mm inter-occlusal clearances are required, T4 could be useful.

How do I use the Dr.Baker’s Prep Gauges to determine the margin width for my crown and veneer preps ?

T 1, Precision CNC milled probe with a contra-angled tip, enables you to measure 0.8 mm along the width of the margin of the preparation finish-line , directly enabling you to visualize whether or not your prep is under or over. Simply sweep the tip of the gauge all around the finish line of the tooth prep. Half the tip diameter will enable to verify margin depths for veneers 0.4 mm [ T1 ] OR 0.5 mm [ T2 ]

It is critically important to accurately obtain 0.7 mm to 0.8 mm of finish-line width in Zirconia crown preps – too narrow and your technician gives you a bulky, over contoured crown. Too wide, you risk weakening the tooth structure and possibly inducing pulpal injury in a vital tooth. Also equally important to obtain a inter occlusal clearance 1.5 mm with the antagonist tooth. Insufficient inter occlusal clearances results in weak prosthesis or having to damage the antagonist. Too much of inter occlusal clearance could result in poor retention of the prosthesis.

But interestingly there are no probes or measuring tools available to the dentist, similar to a periodontal probe for measuring pocket depth. We are talking about measurements that are extremely small 0.5mm – 0.8mm. Even with a magnifying device such as a pair of loupes or telescopes, it is impossible to resolve the difference between 0.5mm / 0.8 mm and 1.0mm, let alone the naked eye!

We conducted a simple pilot survey to test the accuracy of measurement estimations made by experienced clinicians to dental students- they were asked to look at a prep margin and make a suggestion – was the margin width- acceptable / under or over-prepped? The results based on approximations were significantly inaccurate.
